Payment and Insurance Options

For your convenience, we accept various forms of payment, including Visa, Mastercard, Discover, American Express, and Care Credit.

We accept most major medical insurance plans, including Medicare, Medicaid (not MDwise), Anthem, Sagamore, and United. Vision insurance plans (VSP, Eyemed, etc.) are ONLY accepted at Eye Surgeons of Indiana LASIK Center. Contact our office today if you have any questions regarding payment or insurance options available.

Some patients choose to pay for their procedure using a credit card to earn points or use their FSA, HSA, or HRA. LASIK and PRK are eligible expenses for FSA, HSA, and HRA accounts, which means you can use your Flexible Spending Account (FSA), Health Savings Account (HSA), or Health Reimbursement Account (HRA) to cover all or part of the cost of your laser vision surgery.

Financing Options

If you’re interested in financing your procedure, we offer the CareCredit card through our financing partner CareCredit, which can help fit the cost of LASIK into your budget (subject to credit approval; ask your provider or visit carecredit.com for details).
